Administrative Services


**Parents of upcoming 7th and 12th graders, your child will need the following vaccines:**


7th grade:  Required is TDAP & Meningitis.  HPV is recommended but not required.


12th grade: Required is the 2nd Meningitis. Meningitis B is recommended but not required. 


   ***Patients are asked to bring in their Insurance Card(s) Information for all visits.***

Breast and Cervical and Family Planning Clinics: (appointment only)

 Reduced rates on Pap Smears and Breast Exams, for those without insurance*


Immunization Clinic  

  • Every Tuesday from 1pm  - 3:30pm (by appointment)


Shingles/Shingrix can be given if your insurance for Eligible Adults

PCV-21 (pneumonia vaccine) available for those who have not had a prior dose

RSV: available for age 60 and above, who have not had a prior dose, also, available for pregnant women (32-36 weeks pregnant)

Flu: ages 6 mo. and older

COVID 19: ages 12 years and older

  • By Appointment Only!  Normally scheduled on Tuesday mornings Only.

Birth Records:  We do not have birth records. Please contact the Grant County Clerk's office to obtain Birth Certificates. Their phone number is 304-257-4550.
